Website information supports enquiries and is not a diagnosis or individualized medical advice. A clinician should guide testing, interpret results and arrange follow-up. Screening, diagnosis and monitoring serve different purposes.

Requests and appointments

Submitting a form records a request; it does not guarantee approval, funding, a slot or service availability. Confirm the provider, location, preparation, price, additional charges, report turnaround and cancellation or rescheduling arrangements before attending.

Contributions

Read the donation guide. An interest acknowledgement is not proof of payment. Verify project documents, approved payment arrangements and any applicable tax approval before contributing.
