Information provided through forms

Forms request contact details, enquiry text, donation interest or booking details. Healthcare booking may also request age, gender, collection address and appointment information. Provide only what is necessary for the request. Avoid unrelated medical records or financial credentials.

Use and service providers

Details are used to handle requests, arrange appointments, communicate confirmations and maintain operational records. Booking details may be shared with the assigned clinician or diagnostic provider. Email delivery and hosting providers process information needed to operate the service.

Analytics

If configured, website analytics measures general page use and events. Form fields should not be sent to analytics. This update excludes diagnostic detail pages and admin pages from its manual page-view events and omits query strings and fragments. Your browser and network may still share technical connection information with providers.

Retention and privacy requests

Ask the foundation about the retention period, service providers and legal requirements applicable to your records. To request access, correction, deletion or raise a concern, contact the foundation. Identity verification or statutory retention may apply. No fixed retention or deletion timeframe is asserted here.

Sensitive communications

Do not put medical reports, PAN, passwords or banking codes in public comments or ordinary initial enquiries. Ask for the appropriate secure route before sending sensitive documents.
